Care Coordinator II
Location: Remote (McAllen, San Antonio, Austin, TX)
Starting Rate: $17.00/hour (based on experience)
Shift: Monday-Friday, 8:00 AM - 8:00 PM CST
(Overtime available as needed)
Dean's Professional Services is actively seeking a Care Coordinator II to join a collaborative care management team supporting member services and compliance initiatives. This is a fully remote, weekday opportunity offering long-term stability, professional growth, and the ability to make a meaningful impact on member health outcomes through effective care coordination and advocacy.
Responsibilities:
- Act as an advocate for members and serve as a liaison between the Health Plan, providers, and community partners to ensure timely access to care
- Support care management activities by coordinating services outlined in individualized care and service plans
- Conduct member outreach telephonically and, when required, through home visits; document care plans, services, and follow-up activities accurately
- Coordinate ongoing care activities with healthcare providers, community-based organizations, members, and caregivers to accommodate changes in care needs
- Serve as support for member and provider inquiries, requests, and concerns related to care or service plans
- Communicate effectively with care managers, practitioners, and interdisciplinary teams to ensure continuity of care and member satisfaction
- Assist with service assessments and screenings to identify member needs and appropriate interventions
- Maintain accurate and compliant member records in accordance with state, regulatory, and contractual requirements
- Identify gaps in care and make appropriate referrals to Care Managers, Disease Managers, and community resources
- Educate members on available benefits, services, and community resources
- Adhere to established standards of practice, policies, and regulatory guidelines
- Perform other duties as assigned
Qualifications:
- High School Diploma or GED (required)
- 1-2 years of customer service experience in a healthcare environment
- Medicaid and/or Medicare experience (preferred)
- Strong computer proficiency with the ability to navigate multiple systems simultaneously
- Excellent communication, organizational, and time-management skills
- Ability to work independently and effectively in a fully remote environment
