Schedule: Full time Shift: Day (United States of America) Address: 2906 DAVID WALKER DR City: EUSTIS State: Florida Postal Code: 32726 Job Description: Monday-Friday 8:00am-5:00pm *SIGN-ON Bonus of $2,000 for eligible candidates* Responsible for facilitating a seamless health and wellness experience for members. This position establishes trusting, supportive, collaborative relationships with members and their families. Additionally, this role builds relationships with members to navigate and support their health goals, working alongside those involved in their care across all settings and services. In addition, this position proactively communicates with the care team to provide step-by-step guidance for members. Additionally, this role cultivates continuous, supportive relationships with members through emotional responsiveness and informative communication. Individuals in this role are motivated to provide exceptional service and are highly driven to meet the unique needs of a variety of members.

• Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
• Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
• Communicates through various digital channels to support patient care.
• Performs initial health screens and intake for new members.
• Obtains and reviews old medical records to ensure comprehensive patient history.

Knowledge, Skills, and Abilities:

• Knowledge of chronic disease management
• Empathetic nature and a distinct ability to consistently provide top tier service.
• Driven by passionate desire to enhance and improve member outcomes.
• Knowledgeable in member advocacy and navigating complex systems.
• Responsible for managing the member’s entire care plan with the guidance of the RN Case Manager and Provider.

• Graduate of a Medical Assistant Program
