Job Description

As a Community Care Provider, you will deliver strength-based, solution-focused therapeutic support to individuals and families in their natural environments. Responsibilities

  • Provide strength-based, solution-focused therapeutic services to clients in community settings.
  • Deliver consumer-driven and family-oriented care, tailored to each client’s unique needs and environment.
  • Offer individual, client-centered Psychosocial Skills Rehabilitation (PSR) services.
  • Develop a thorough understanding of each client’s strengths, diagnosis, treatment goals, and treatment plan.
  • Assess clients for strengths, resources, and goals within the first 30 days of treatment and provide ongoing strength-based planning and support.
  • Assist with 90-day treatment reviews and participate in collaborative meetings with staff, agencies, and community supports as needed.
  • Provide crisis intervention services in accordance with agency policies and procedures.
  • Communicate regularly about client status, including changes in mood, functioning, medical issues, and any crisis or safety concerns.
  • Maintain accurate, timely, and effective clinical documentation for all client interactions and services provided.
  • Attend required office trainings and regular clinical supervision sessions.
  • Comply with all safety rules and agency policies.
  • Complete required annual training hours (20 hours for full-time, 16 hours for part-time).
  • Actively participate in evaluations conducted by Rehabilitation Services.

Qualifications

  • Master’s degree from an accredited university or college in Counseling, Social Work, Psychology, or Sociology.
  • Willingness to learn and utilize computer systems for documentation and billing purposes.
  • Strong communication skills and ability to work collaboratively with clients, families, and team members.
  • Organizational skills to manage documentation and meet deadlines.
  • Commitment to ongoing professional development and training.
