Job Description

Accelerated Home Health Care is hiring a Medical Social Worker (MSW) to provide in-home psychosocial support to patients and families.

Key Responsibilities:

• Conduct assessments of psychosocial needs.
• Provide counseling and emotional support.
• Connect patients and families to community resources.
• Document interventions and communicate with the care team.
• Follow care plans and regulatory requirements.

Minimum Certification & Licensing Requirements:

• MSW degree and state licensure.
• Completion of agency onboarding/credentialing.
• Up-to-date TB test and reliable transportation.

Preferred Qualifications:

• Experience in home health or hospital settings.
• Strong interpersonal and problem-solving skills.
• Ability to work independently in a home-based setting.

Working Conditions:

• In-home visits with travel between patients.
• Flexible schedule, including occasional weekends.
• Must be able to handle in-home work safely and independently.
