Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Provide physical therapy services within a home-based hospice care setting, following the prescribed plan of care and documenting progress.
- Monitor patient responses to therapy, adjust interventions as needed, and communicate changes to the care team.
- Collaborate with nurses, social workers, and other professionals to support comprehensive patient goals.
- Maintain accurate documentation of evaluations, treatment plans, and outcomes in a timely manner.

Requirements
- Ability to work in a home-based hospice setting and participate in a day shift schedule.
- Strong communication and collaboration skills for interdisciplinary care.
- Demonstrated reliability and ability to manage caseload in a travel assignment.
- Eligibility to work in the United States.
- Willingness to commit to a 13-week assignment.
